APPLICATION FOR REGISTRATION OF CORPORATE NAME FILING INSTRUCTIONS REGISTRATION OR RENEWAL If the corporation is applying for registration of corporate name, check appropriate block. If the corporation is applying for renewal registration of corporate name, check appropriate block. CORPORATE NAME Use the exact name of the corporation as filed in its state of incorporation. The corporate name must contain the word "corporation," "incorporated," "company," or "limited," or an abbreviation thereof. DATE OF INCORPORATION The date of incorporation is the date articles of incorporation were filed with the secretary of state or other official having custody of the corporate records. NATURE OF BUSINESS The corporation must give a brief description of the nature of the business in which it is engaged. CERTIFICATE OF EXISTENCE A certificate of existence or document of similar import (for example, certificate of "good standing") must accompany this application. The certificate must be an original and must not be older than six (6) months. A copy of the certificate is not acceptable. Please contact the secretary of state's office of the domiciled state for a certificate of existence. WHO MAY SIGN An officer or the chairman of the board of directors must sign the application for registration of corporate name. Type or print name, title and the street address of the corporation. NUMBER OF COPIES Submit one originally signed and one exact or conformed copy. (May be a photocopy) One filestamped copy will be returned to the corporation as evidence of filing. FILING FEE The filing fee for registration or renewal of corporate name is $36.00. Your check should be made payable to the "Kentucky State Treasurer".
