NON-PROFIT CORPORATION NAME The non-profit corporation name provided on the articles of dissolution must read exactly as stated in the most recent articles. This can be found using the organization search tool located on the Secretary of State website at www.sos.ky.gov. REQUIREMENTS FOR DOCUMENTS TO BE PROPERLY FILED The articles of dissolution must be signed by a member or manager. Documents must be submitted either typed or printed in black ink. PLAN OF DISTRIBUTION

If you need to file a plan of distribution, you should not use this form, but rather submit a drafted executed copy of the articles of dissolution.
ADDITIONAL ARTICLES OF DISSOLUTION OR NEED TO MODIFY THE EXISTING FORM If this form does not comply with the articles of dissolution that you wish to file, please disregard this form and send a drafted executed copy of the articles of dissolution according to KRS 273.313 to the address below. DOCUMENT DELIVERY All articles of dissolution will be sent to the return address on the outer envelope of the submitted articles. If no address is found, the formation documents are sent to the principal office on file. If the officer wishes for correspondence from the Office of the Secretary of State to be sent to an individual or corporation address other than those above, a request must be submitted in writing affirming that request. All other communication and notification shall follow the process prescribed in Kentucky Revised Statute.
