If the court has ordered you to sell or turn in your firearms, you may use this form to prove to the court that you have obeyed its orders. You must deliver your weapons unloaded. When you deliver your weapons, ask law enforcement or the licensed gun dealer to complete item 4 or 5 and item 6 . After the form is signed, file it with the court clerk. Keep a copy for yourself.
