COMES NOW PROBATION OFFICER Michael G. Wimmer presenting an official report on
Timothy Wayne CLATONE who was committed to the Bureau of Prisons for 10 months May 9, 2002, by
the Honorable James A. Teilborg presiding in the District Court of Arizona. A 2-year period of supervised
release was imposed, and supervision commenced upon the offender's discharge from imprisonment
January 12, 2004. In addition to the general terms and conditions adopted by the Court in General Order
99-9, the offender was ordered to comply with the following special conditions:
1. Defendant shall participate as instructed by the probation officer in a program of substance abuse
treatment which may include testing for substance abuse. He shall contribute to the cost of treatment
in an amount to be determined by the probation officer.
2. Defendant shall submit to search of person, property, vehicles, business, and residence to be
conducted in a reasonable manner and at a reasonable time by, or at the direction of, the probation
officer.
Defendant shall complete 100 hours of community senrice at the rate of not less than 20 hours per
month as approved and directed by the probation officer, to be completed within six (6) months from
release from custody.
4. Defendant is prohibited from owning, maintaining or using a firearm.
Timothy Wayne Clatone was convicted of Possession of Stolen Mail, a Class D felony.
RESPECTFULLY PRESENTING PETITION FOR ACTION OF COURT FOR CAUSE
AS FOLLOWS:
Allegation A: Violation of Standard Condition #3: You shall report to the Probation Office as directed
bythe Court or probation officer, and shall submit a truthful and complete written report within the Hrst
live days of each month.
1. The defendant failed to report to the Probation Office December 16, 2005, as directed by the
supervising probation officer. A Grade C violation.
2. The defendant failed to submit written reports for the months of November and December
2005. A Grade C violation.

December 7; December 13; and December 29, 2005. A Grade C violation.
AFFIDAVIT AND PETITION PRAYING THAT THE COURT WILL ORDER A WARRANT BE ISSUED
In conformance with the provision of 28 U.S.C. § 1746, I declare, under penalty of perjury, that the
foregoing is true and correct to the best of my knowledge. Based on the information presented that the
offender has violated conditions of supervision, I am petitioning the Court to issue a warrant.
