The undersigned, certifies under penalty of perjury the following: I I am a registered Private Process Server in the
county of MARICOPA, I am an Officer of the Court and I am qualified pursuant to RCP4 (d), 4 (e) and 45 (d), to
serve process in this case.
On January 6, 2006 I received a Subpoena Duces Tecum In a Civil Case to Jerry Lilly; a Notice of Videotaped
Deposition and Letter dated January 6, 2006 from attomey Adam Lang of the law firm of Snell & Wilmer. In
each instance I personally served one copy each of the above listed documents, on those named below in the
manner and at the time and place shown, that all services except where noted were made within Maricopa County,
Arizona. _
I personally served wife Sharon Lilly on January 9, 2006 at 13:08 pm, on behalf of Jerry Lilly, at their place of
residence, which is located at 935 West Laredo Avenue, Gilbert, AZ 85233. Mrs. Lilly said she was authorized to
accept for her husband Jerry Lilly and she would be sure he received the subpoena.. Sharon Lilly is described as
female, approximately 35 years of age, of 5' 7" height, l40# weight, and brown hair. I certify under the penalty of
perjury the foregoing is true and correct.
