COMES NOW the defendant, representing myself, who respectfully requests that

this Honorable Court release my passport, which was surrendered tothe Clerk of the Court
14
on June I5, 2004 as a condition of release on personal recognizance.

I. On June 15, 2004 , I entered a plea of guilty to one count of healthcare fraud in
16
violation of 18 U.S.C. §I347, a Class D felony.

2. On June I5, 2004, I surrendered my passport to the Clerk ofthe Court as a
16
condition of release on personal recognizance.

3. On January 3, 2005, I appeared in Court for my sentencing.

4. I have complied with all ofthe conditions of release on personal recognizance.
