Instructions for completing Petition For Validation Of Adoption Finalized In A Foreign Country

Form use. If an adoption has been finalized in another country, the adoptive parent may be able to file this petition to ask the court to validate the adoption in NH. Page 1. Top part of form. ·COURT NAME: Enter the name of the county probate court where the document will be filed. (example: Belknap County Probate Court; Rockingham County Probate Court). ·CASE NAME: Enter the birth name of the child who will be adopted (example: Adoption of John Adams; Adoption of Susan Jones). ·CASE NUMBER: Leave blank if not yet assigned by court OR fill in case number if it is known. Numbered part of form. 1. Petitioner Name is the name of the person(s) filing the petition. Enter the name(s) of each person and their telephone numbers. Enter the complete mailing address with zip code. Complete this section ONLY if the petitioner has an attorney assisting with this case. Enter his or her name, telephone number and complete mailing address including zip code. The Child Birth Name is the name given to the child at birth. Enter the name of the child on the line provided. Also, enter the child's date of birth and place of birth including the town or city and country. Check off the appropriate box to indicate the document you are submitting attached to this form that validates this international adoption.

Signature section On the line provided prior to the signatures, as part of the request to validate this adoption, enter your desired name for the child. Check off one box to indicate whether or not a hearing is requested. You will sign the form on the Petitioner's Signature line, and date it in the appropriate space to the left. If there are two petitioners, both petitioners must sign and date the form. Decree This section will be completed by the judge once the document is filed with the court and reviewed in detail by the judge.
