Instructions for completing Certification of Copies to Parties
(NHJB-2148-P)

Form use. Under Probate Court Rule 21, any person filing a motion, objection, answer, inventory or correspondence with the probate court must furnish copies to all attorneys, parties, and persons beneficially interested in the case. If such certification is not included on the form filed with the court, this form may be used. Top part of form ·COURT NAME: Enter the name of the county probate court where the document will be filed. (example: Belknap County Probate Court; Rockingham County Probate Court). ·CASE NAME: Enter the name of the case (example: Estate of John Adams; Guardianship of Susan Jones). ·CASE NUMBER: Leave blank if not yet assigned by court OR fill in case number if it is known. Main part of form · In the first sentence, on the blank line, insert the name of the document that has been sent to all interested parties.

Signature section Sign the form on the SIGNATURE line, and date it in the appropriate space to the left. If your attorney sends the forms, your attorney may sign this form.
