I, ______________________________________, hereinafter Parenting Plan Home Study Evaluator, agree to perform custody evaluations for West Virginia Supreme Court fee waiver-approved families. Eligibility shall be determined by the Family Court ordering the evaluation. The term of service begins on July 1, 2003. Service can be terminated by either party at any time. The total amount billed per case may not exceed seven hundred fifty dollars ($750.00), regardless of how many parties are involved or interviewed.. This agreement does not create an employer/employee relationship between EVALUATOR and COURT. EVALUATOR acts only as an independent contractor of the COURT.
