________________________ requests an exemption from payment of the Electronic Public Access Fee ("PACER User Fee") which is prescribed by the Judicial Conference of the United States as set forth in 28 U.S.C. § 1930. In support of this Motion For Exemption From Payment of the PACER User Fee the movant states as follows: 1. Movant is (must attach supporting affidavit):

2. 9 Exemption from payment of the PACER User Fee is necessary in order to avoid unreasonable burdens and to promote public access to information. As specific grounds for the exemption the movant states the following (state specifically how exemption from the fee is necessary in order to avoid unreasonable burdens and to promote public access to information): ___________________________________________________________________ ___________________________________________________________________ ___________________________________________________________________ ___________________________________________________________________ ___________________________________________________________________ ___________________________________________________________________ ___________________________________________________________________ ___________________________________________________________________ ___________________________________________________________________ ___________________________________________________________________ ___________________________________________________________________ 3. 9 Movant agrees that all information, reports, documents and other data obtained using a feeexempt PACER account will not be sold for profit or otherwise exchanged for value. 4. 9 Movant requests exemption as a bankruptcy case trustee and is an attorney: a. 9 Movant agrees that all information, reports, documents and other data obtained using a fee-exempt PACER account will be used only in connection with the movant's duties as a bankruptcy trustee and not for other legal work. b. 9 Movant agrees to establish a separate fee-based PACER account for non trusteerelated work and will use the appropriate PACER account for that work. 5. 9 Movant requests exemption other than as a bankruptcy trustee (i.e.; movant is an attorney, a not-for-profit organization, a researcher, or indigent): a. 9 Movant agrees that all information, reports, documents and other data obtained using a fee-exempt PACER account will be used only in connection with the movant's activities which qualify for the exemption and not for activities which are unrelated to the exemption. b. 9 Movant agrees to establish a separate fee-based PACER account for activities Page 2 of 4

which are not exemption-related will use the appropriate PACER account for those activities.

6. 9 Movant acknowledges that exemption from payment of the PACER User Fee will be granted for a maximum term of: a. 36 months for a bankruptcy case trustee or upon completion of all duties for which the case trustee has been appointed, whichever comes first; b. 36 months for courts; c. 12 months for all other PACER users. 7. 9 Movant acknowledges that the exemption from payment of the PACER User Fee may be revoked at any time at the discretion of the Court. 8. 9 Movant acknowledges that movant may not knowingly permit or cause to permit the PACER User Fee exemption to be used by anyone other than an authorized agent of the movant. Furthermore, movant acknowledges that the agent's use of the exemption shall be only in connection with the exemption-related activities for which the movant was granted the exemption, and that the agent is subject to the movant's restrictions under Section 2(c)-(e) of the "Guidelines For Motion For Exemption From Payment of the PACER User Fee." 9. Indicate which of the following documents are attached in support of the Motion: a. 9 Affidavit regarding current status as a bankruptcy case trustee. b. 9 Affidavit of officer, director, or other authorized individual stating current status of movant as a not-for-profit organization qualified for tax-exempt status under § 501(c)(3) of the Internal Revenue Code. c. 9 Affidavit of court clerk or court administrator. d. 9 Affidavit regarding current status as an individual researcher associated with an educational institution. e. 9 Affidavit regarding indigency. f. 9 Other: _____________________________.
