Th I defendant waived indictment and pled guilty to a one count substitute infcIrmation after a plea of not guilty to
an Indictment. Accordingly, the defendant is adjudged guilty of count one, whch involves the following offense:

Th sentence is imposed pursuant to the Sentencing Reform Act of 1984. I

Th defendant is hereby sentenced to the custody of the United States Attorneg General or his duly authorized I
rep esentative to be imprisoned for a term of 48 months. The court recommen s to the Bureau of Prisons that I
the defendant be allowed to participate in the 500 Hour Comprehensive Drug T eatment Program at his earliest I
op ortunity. Upon release from custody, the defendant will be on supervised elease for a term of 8 years with I
the following special conditions: (1) the defendant will participate in a program of substance abuse monitoring,
co nseling, and treatment under the direction of the Probation Office; the defe dant will pay all, or a portion of,
the costs associated with any such treatment based on his ability to pay, in an amount to be determined bythe
Pr ation Office; (2) the defendant will participate in an employment readiness program or vocational training
un er the direction of the Probation Office; and (3) the defendant will seek andImaintain full-time employment.
It I further ordered that the defendant will notify the United States Attorney fo this district within 30 days of any
ch nge of name, residence, or mailing address until all fines, restitution, costs and special assessments imposed
by his judgment are fully paid. In addition to the special conditions of supervi ed release imposed above, it is
her by ordered that the general conditions of supervised release set out on th reverse side are also imposed.

1 Whilm on supervised release or probation, the defendant shall abide by the following
1 cond1tions in addition to any special conditions set by the court:
1 1) 1The defendant shall not commit another federal, state1 or local crime;
2) 1 The defendant shall report in person to the probation office in the district
to w1ich the defendant is released within 72 hours of release from the custody of
the =ureau of Prisons; and shall submit a truthful and complete written report 1
with n the first 5 days of each month; 1
3) The defendant shall pay any such restitution and fines that remains unpaid at
the •ommencement of the term of supervised release; 1
1 4) The defendant shall not leave the judicial district without the permission of
the •ourt or probation officer;
1 5) The defendant shall answer truthfully all inquiries by the probation officer

7) The defendant shall work regularly at a lawful occupa1ion unless excused by 1
the •»obation officer for schooling, training, or other acceptable reasons;

in re»idence or employment;
9) The defendant shall not possess a firearm or destructive device. 1
10) The defendant shall refrain from excessive use of alcdhol and shall not -
purch=se, possess, use, distribute, or administer any narcotic or other controlled
subst1nce, or any paraphernalia related to such substances,1except as prescribed by

11) 1The defendant shall not frequent places where controlqed substances are 1
illegmlly sold, used, distributed, or administered; 1
12) 1The defendant shall not associate with any persons engaged in criminal 1
activ1ty, and shall not associate with any person convicted1of a felony unless
grantwd permission to do so by the probation officer; 1 1
13) 1The defendant shall permit a probation officer to visnt him/her at any 1
time mt home or elsewhere and shall permit confiscation of any contraband observed
in pl1in view by the probation officer. 1 1
14) 1The defendant shall notify the probation officer withnn 72 hours of being 1
arres·ed or questioned by a law enforcement officer;
15) 1The defendant shall not enter into any agreement to ach as an informer or 1
