WELL PERMIT INFORMATION, PERMITS APPROVED PURSUANT TO SECTIONS 37-90-107(7), C.R.S. (DENVER BASIN AQUIFER WELLS LOCATED IN DESIGNATED GROUND WATER BASINS) PLEASE NOTE CAREFULLY THE CONDITIONS OF APPROVAL ON THE ATTACHED COPY OF YOUR PERMIT. THE CONDITIONS MUST BE COMPLIED WITH IN ORDER FOR THE PERMIT TO BE VALID. THE WELL CONSTRUCTION WORK MUST BE COMPLETED PRIOR TO THE EXPIRATION DATE OF THE PERMIT. THE PERMIT EXPIRATION DATE IS ONE YEAR FROM THE DATE ISSUED. Evidence of well construction must be submitted to this office in the form of a well construction report from the respective contractors showing that construction was completed prior to the expiration date of the well permit. The well permit number is located in the upper right corner and the expiration date is located in the lower right corner. At the discretion of the Commission, the expiration date may be extended once for up to one year if good cause is shown. If an extension of time is necessary to construct the well, you must file a written request with this office prior to the expiration date. The request must be received with a $60 filing fee and must state why the well has not been completed, must include an estimate of time required to complete the well, and must specify the length of extension you desire, not to exceed one year. A NOTICE OF COMMENCEMENT OF BENEFICIAL USE (FORM NO. GWS-19) MUST BE FILED WITH THE DIVISION OF WATER RESOURCES BY THE WELL OWNER WITHIN 30-DAYS AFTER FIRST COMMENCEMENT OF USE. Please retain the enclosed copy of this form and file it with this office when the well is
first put to use.

ADDITIONAL INFORMATION REGARDING METERING AND GEOPHYSICAL LOGGING REQUIREMENTS FOR THIS WELL ARE FOUND ON THE SECOND PAGE OF THIS FORM. THE WELL MUST BE CONTRUCTED AND THE PUMP INSTALLED BY CONTRACTORS WITH CURRENT LICENSE(S) ISSUED BY THE STATE OF COLORADO. The well construction report must be submitted to the
office of the State Engineer within sixty (60) days of completion of the work or within seven days after the expiration date of the permit, whichever is earlier. The pump installation report must be submitted within sixty days of completion of the pump installation work. Your contractor must provide you with a copy of the work reports they have filed with the State Engineer. The Well Construction and Test Report, Form No. GWS-31, and the Pump Installation and Test Report, Form No. GWS-32, are available from the Division of Water Resources Offices and the web site, www.water.state.co.us . (Note: Wells in some areas may be constructed by the well owner. Contact this office for more information.) The original permit is on file in the Denver Office Records Section. You have been provided with at least two copies of the well permit. One copy is for your records. The second is for the pump installation contractor. Permit copies have also been provided to the well construction contractor indicated in the application. If you did not indicate a well construction contractor on the application, four copies of the permit are enclosed. You may make additional copies of your permit copy for a well construction contractor if different from the one indicated in your application. Additional permit copies may be obtained from the Records Section, (303) 866-3447, for a fee of $3.00. Any change of mailing address or ownership must be reported to the State Engineer by the new owner on a Change in Ownership/Address form, GWS-11. Metering and Reporting Withdrawal Requirements from the Denver Basin Aquifer Wells Permitted Pursuant to Section 37-90-107(7), C.R.S.: In order to administer these wells, the State Engineer shall require the permittee to
install and maintain a totalizing flow meter to measure all withdrawals from the well. As a minimum, the meter shall be installed according to manufacturer's recommendations and shall contain sufficient recording digits to assure that "roll over" to zero does not occur within three years. The meter shall be maintained by the permittee so as to provide continuous, accurate records of withdrawal. If the meter is not operational, the well shall not be pumped. The State Engineer may approve a substitute totalizing measuring device if it is capable of recording or can be used to calculate the total volume pumped. The permittee shall report the amount of withdrawal from the permitted well as reasonably required by the State Engineer.

Geophysical logs are required for all new wells permitted pursuant to Section 37-90-107(7), C.R.S. unless there
is an existing geophysical log from that aquifer located within 1,320 feet of the proposed well which is acceptable to the State Engineer and is representative of the aquifer conditions at the location of the permitted well. The permittee will be responsible for obtaining and submitting a copy of the existing geophysical log unless it is currently on file in the State Engineer's Office. If the new well is geophysically logged, the permittee is responsible for obtaining and submitting those required geophysical logs to the State Engineer's Office as follows: 1) The minimum requirements will be for both resistivity and spontaneous potential (SP) logs, which must be obtained prior to the installation of the well casing. 2) The resistivity log must record values at approximately one-and three-foot depths of investigation. 3) The vertical scale shall be either one inch equals twenty feet, or one inch equals fifty feet. 4) If additional types of logs, including gamma, neutron, porosity, sonic, single point resistance, and caliper, are obtained, copies of these logs shall also be submitted to the State Engineer's Office. 5) The entire set of logs must be submitted to the State Engineer's Office within sixty (60) days following collection of the data or within seven days after the expiration date of the well permit, whichever is earlier. The log shall be identified by the well permit number. 6) If a permittee, or an applicant drilling a test hole, desires to use other geophysical logs to meet the minimum requirements, they must apply in writing to , and obtain approval from the State Engineer's Office prior to well construction. 7) In the event that the data provided contains trade secrets, privileged information, or confidential commercial, financial, geological or geophysical data, the State Engineer shall deny the right of inspection of such data to any person and keep the data confidential pursuant to C.R.S. 24-72-204(3)(a)(IV), provided the applicant notifies the State Engineer in writing requesting such protection. The State Engineer may require the applicant to submit the geophysical well logs prior to the casing of the production well in order to determine the appropriate producing intervals. In such a case, the State Engineer shall inform the applicant of the appropriate producing interval within two working days of receipt by the State Engineer of the geophysical logs.
