NOTICE TO PARENTS As a parent of a minor child, you are obligated by Ohio law and local rule of Court to notify the Butler County Domestic Relations Court Case Management Office whenever you intend to change your address. You must notify the court of each new address by completing the enclosed Notice of Intent to Relocate Form (C13 or C13A), even if your former spouse is aware of the move, and already knows what your new address will be. However, if your former spouse is aware of the move and wishes to waive further notification by the court, he or she may sign the Waiver of Court Notification Form (C19), which is also enclosed. Both the notice and the waiver must be completed and returned to the court prior to any change of residence. Unless certain exceptions apply, the court will send your new address to your former spouse. If you feel circumstances warrant withholding your address from the former spouse, you must complete the enclosed Supplemental Affidavit (C20) and return it with the Notice of Intent to Relocate Form.
