Using a Federal Identification Number for the IFTA/SFU Account Number
The purpose for this notice is to protect your Social Security Number from being distributed through IFTA/SFU credentials. If your IFTA/SFU account number currently is a Federal Identification Number, assigned by the IRS, you may ignore this notice. Look on your IFTA/SFU return to see if the IFTA/SFU account number is a Social Security Number. If it is a Social Security Number and if you have a current Federal Identification Number used in your motor carrier activity, complete the IFTA/SFU Account Number Change Form below using that Federal Identification Number. If it is a Social Security Number and you do not have a Federal Identification Number to use in your motor carrier activity, contact the IRS to obtain a Federal Identification Number by calling 1-800-829-4933 or go to the internet at www.irs.gov/businesses and click on Employer ID Numbers. Once you get your Federal Identification Number, then complete the IFTA/SFU Account Number Change Form below. Please attach a copy of the federal form or document that verifies your Federal Identification Number. Return the IFTA/SFU Account Number Change Form to the Utah State Tax Commission, Taxpayer Services, 210 N 1950 W, SLC, Utah 84134-7000. You may also fax the IFTA/SFU Account Number Change Form to (801) 297-6357. Once the IFTA/SFU Account Number Change Form is received in our office, the Utah State Tax Commission will update your IFTA/SFU account number, using your new Federal Identification Number. You will be issued a new IFTA/SFU license with the new federal ID account number. You should make a copy of this license to replace the old license. You must file all IFTA/SFU information using this new Federal Identification Number as your IFTA/SFU account number.
