ORDERS AND ADVISEMENT TO MINOR TRANSFERRED TO SACRAMENTO COUNTY FOR SUPERVISION [Welfare and Institutions Code § 775 or 1766(b)(5)] The Juvenile Court has ordered the following: 1. The Sacramento Juvenile Court has accepted jurisdiction of your case from another County or the California Department of Corrections and Rehabilitation, Division of Juvenile Facilities (DJF). 2. You must obey the Sacramento County General Conditions of Probation as stated on the reverse side of this document. These conditions replace any conditions previously imposed by the Transferring County or DJF which are in conflict with the Sacramento County General Conditions of Probation. 3. Any of the conditions imposed by the Transferring County or DJF which are not in conflict with the Sacramento County General Conditions of Probation are still in effect. 4. You are to report within five (5) days to the Probation Department office located at (circle one): 10361 Rockingham Drive, Suite 600 Sacramento, CA 3201 Florin-Perkins Road Sacramento, CA

5. The court has referred your case to the Sacramento County Probation Department for a review of the probation conditions which were previously imposed by the Transferring County or DJF. The Sacramento County Probation Department may submit proposed revised and additional probation conditions that may be more suitable to you as a resident of this county because our terminology may be different than that used in that county or by DJF because of new information about you that became known after you were put on probation in the Transferring County or released on parole by DJF. 6. A Modification Hearing has been set for ________________________________. At this hearing, the Juvenile Court will review the conditions recommended by the Probation Department and will order any new probation conditions that the Court finds should apply to you. Your attorney will be present at the hearing. Your attorney will help you decide whether you need to appear at that hearing. 7. Whether or not you appear for the Modification Hearing, you are ordered to go over the full set of probation conditions with the probation officer assigned to your case at your next appointment to see him/her after the Modification Hearing is held.
