I, Jude P. Damasco, am over eighteen years of age and state the following facts which I know of my personal knowledge. 1. I am the owner of Damasco & Associates ("Damasco"), which is located at 505

Sansome Street, Suite 1701, San Francisco, CA 94111. I am a certified public account. 2. Damasco was appointed as Tax Administrator for the SEC v. Qwest Communications

Settlement Fund in the Order Appointing Tax Administrator, Civil Action No. 04-D1267(MJW), filed June 8, 2005. 3. As Tax Administrator, Damasco has determined that SEC v. Qwest Communications

Settlement Fund owes $405,200 in tax liability for the second quarter of 2005. 4. A check should be made payable in the amount of $405,200 to Damasco & Associates

and sent to Damasco & Associates, 505 Sansome Street, Suite 1701, San Francisco, CA 94111. 5. The check will be deposited to a trust account from which the tax payment will be

made. Therefore, the Employer Identification Number ("EIN") that should be written on the check is 20-2883185 because it is for tax payment on behalf of SEC v. Qwest Communications Settlement Fund.
