COMES NOW PROBATION OFFICER Elizabeth P. Simons presenting an official report on
Steven Anthony Murillo who was committed to the Bureau of Prisons on October 15, 2001, by the
Honorable Susan R. Bolton presiding inthe District Court ofArizona. A three year period of supervised
release was imposed, and supervision commenced upon the offender's discharge from imprisonment
on January 23, 2006. In addition to the general terms and conditions adopted by the court, the offender
was ordered to comply with the following special conditions:
1. You shall participate as instructed by the probation officer in a program of substance abuse
treatment which may include testing for substance abuse. You shall contribute to the cost of
treatment in an amount to be determined by the probation officer.
2. You shall submit to search of person, property, vehicles, business, and residence to be conducted
in a reasonable manner and at a reasonable time by, or at the direction of, the probation officer.
Steven Anthony Murillo was convicted of Ct. 2: Possession With Intent to Distribute More than 100
Kilograms of Marijuana, a Class B felony in violation of 21 USC § 841(a)(1) and (b)(1)(B)(vii). He was
originally sentenced to 84 months in prison.
RESPECTFULLY PRESENTING PETITION FOR ACTION OF COURT FOR CAUSE
AS FOLLOWS:
A. Violation of Standard Condition 1: "You shall not commit another federal, state, or local
crime during the term of supervision."
· On or about August 1, 2006, Murillo committed the crimes of Possession of Narcotic Drugs, a
Class 4 felony in violation ofA.R.S. § 13-3408A; and Possession of Drug Paraphernalia, a Class
6 felony in violation ofA.R.S. § 3415A, as evidenced by Phoenix Police Departmental Report No.
2006—61451775, and Maricopa County Superior Court Docket No. CR2006—146057-001 DT.
Grade B violation. §7B1.1(a)(2).
B. Violation of Standard Condition 3: "You shall report to the Probation Office as directed by
the Court or probation officer, and shall submit a truthful and complete written report
within the first five days of each month."
Murillo failed to report as directed to the probation office on August 15, 2006. Grade C violation.

C. Violation of Standard Condition 3: "You shall report to the Probation Office as directed by
the Court or probation officer, and shall submit a truthful and complete written report

ll/Iurillo failed to submit written monthly reports for July and August 2006. Grade C violation.
§7B1 .1(a)(3).
D. Violation of Standard Condition 7: "You shall notify the probation officer at least ten days
prior to any chan e of residence or employment."
lvlurillo failed to notify the probation officer of a change in residence. On August 4, 2006, this
officer spoke with IVIuriIIo's father who stated he had not returned home since his release from jail
on August 2, 2006. His current whereabouts are unknown. Grade C violation. §7B1.1(a)(3).

AFFIDAVIT AND PETITION PRAYING THAT THE COURT WILL ORDER A WARRANT BE ISSUED
ln conformance with the provision of 28 U.S.C. § 1746, I declare, under penalty of perjury, that the
foregoing is true and correct to the best of my knowledge. Based on the information presented that the
offender has violated conditions of supervision, I am petitioning the Court to issue a warrant.
