county of MARICOPA, I am an Officer of the Court and I am qualified pursuant to RCP4 (d), 4 (e) and 45 (d), to
serve process in this case.
On January 11, 2006 I received a Subpoena Duces Tecum In a Civil Case to Beazer Homes Sales Inc. and a Notice
of Videotaped 30(b)(6) Deposition from attomey Dan Goldfine #018788 ofthe law firm of Snell & Wilmer. In ·
each instance I personally served one copy each of the above listed documents, on those named below in the
manner and at the time and place shown, that all services except where noted were made within Maricopa County,
Arizona.
I personally served Amy St. Lawrence on January 11, 2006 at 2:43 pm, on behalf of Beazer Homes Sales, Inc.,
Statutory Agent, at her place of business, Corporation Service Company, which is located at 2338 West Royal
Pahn Road, Suite J, Phoenix, AZ 85021. Amy St. Lawrence is described as female, approximately 25 years of
age, of 5' 6" height, 140# weight, and brown hair. I certify under the penalty of perjury the foregoing is true and
correct.
