Petition to Revoke Supervised Release I
COMES NOW PROBATION OFFICER R. Kelly Cutright presenting an official report on Cody Lee CAMPILLO who was
committed to the Bureau of Prisons upon revocation on December 13, 2005, by the Honorable Paul G. Rosenblatt presiding
in the District Coun of Arizona. A 30 month period of supenrised release was imposed, and supervision commenced upon the
offenders discharge from imprisonment on May 22, 2006. In addition to the general terms and conditions adopted by the court,
the offender was ordered to comply with the following special conditions:
1. You shall participate as instructed by the probation officer in a program of substance abuse
treatment which may include testing for substance abuse. You shall contribute to the cost of
treatment in an amount to be determined by the probation officer.
2. You shall submit to search of person, property (including but not limited to computer, electronic
devices, and storage media), residence, office, or vehicle to a search conducted by a probation
officer, at a reasonable time and in a reasonable manner.
3. You shall abstain from all use of alcohol or alcoholic beverages.
4. You shall submit to evaluation and/or treatment in an approved domestic violence prevention
program as directed by the probation officer. You shall contribute to the cost of treatment in an
amount to be determined by the probation officer.
Cody Lee CAMPILLO was convicted of Possession with Intent to Distribute Marijuana, a Class C felony.
RESPECTFULLY PRESENTING PETITION FOR ACTION OF COURT FOR CAUSE AS FOLLOWS:
A. Violation of Standard Condition Number 3: "You shall report to the Probation Office as
directed by the Court or probation officer, and shall submit a truthful and complete written
report within the first five days of each month."
Campillo failed to submit a written report within the first five days of April, May, June, July and
August 2007. Pursuant to §7B1.1(a)(3)(B), this is a Grade C violation.
B. Violation of Standard Condition Number 4: "You shall answer truthfully all inquiries by the
probation officer and follow the instructions of the probation officer."
Campillo failed to submit urine samples twice per month since March 19, 2007, as instructed by
the probation officer. Pursuant to §7B1.1(a)(3)(B), this is a Grade C violation.

in conformance with the provision of 28 U.S.C. § 1746, I declare, under penalty of perjury, that the
foregoing is true and correct to the best of my knowledge. Based on the information presented that the
offender has violated conditions of supervision, I am petitioning the Court to issue a warrant.
