Petition to Revoke Supervised Release T-
COMES NOW PROBATION OFFICER Matthew Cary presenting an official report on Kristina
Nicole Ingles who was committed to the Bureau of Prisons on August 13, 2007, by the Honorable
Lawrence O. Anderson presiding in the District Court of Arizona. A 12 month period of supervised
release was imposed, and supervision commenced upon the offender’s discharge from imprisonment
on December 21, 2007. In addition to the general terms and conditions adopted by the court, the
offender was ordered to comply with the following special conditions:
1. You shall participate as instructed by the probation officer in a program of substance abuse
treatment which may include testing for substance abuse. You shall contribute to the cost of
treatment in an amount to be determined by the probation officer. _
2. You shall provide the probation officer access to any requested financial information.
3. You shall participate in a mental health program as directed by the probation officer which may
include taking prescribed medication. You shall contribute to the cost of treatment in an amount
to be determined by the probation officer.
4. You shall reside and participate in a residential re-entry center for 4 months, unless discharged
earlier by the probation officer.
5. You shall not contact the following person, Jeremy Arnold, and the probation officer will verify
compliance.
6. You shall abstain from all use of alcohol or alcoholic beverages.
7. You shall resolve all outstanding citations from the City of Glendale within a reasonableperiod of I
time upon release from custody.
Kristina Nicole Ingles was convicted of Theft of Government Property.

RESPECTFULLY PRESENTING PETITION FOR ACTION OF COURT FOR CAUSE
AS FOLLOWS: .
A. Violation of Special Condition No. 1: You shall participate as instructed by the probation
officer in a program of substance abuse treatmentwhich may include testing for substance
abuse. You shall contribute to the cost of treatment in an amount to be determined by the
probation officer.
Ingles has failed to participate in substance treatment as directed. She began participating in
substance abuse treatment with Northern Arizona Substance Abuse Sen/ices on March 31, 2008.
Ingles has not attended any treatment sessions since May 21, 2008.
Grade C Violation, 7B1.1(a)(3)(b).
B. Violation of Special Condition No. 1: You shall participate as instructed by the probation
officer in a program ofsubstance abuse treatmentwhich may include testing for substance
abuse. You shall contribute to the cost of treatment in an amount to be determined by the
probation officer.
Ingles failed to report for urine testing as directed on 5/27/08, 6/2/08, 6/30/08, 7/3/08 and 7/7/08.
Grade C Violation, 7B1.1(a)(3)(b).
C. Violation of Standard Condition No. 9: You shall not purchase, possess, use, distribute or
administer any narcotic or other controlled substance as defined in section 102 of the
Controlled Substance (21 U.S.C. § 801) or any paraphernalia related to such substances,
without a prescription by a licensed medical practitioner. Possession of controlled
substances will result in mandatory revocation of your term of supervision.
1. Ingles used methamphetamine on or about 5/17/08 as evidenced by the admission form
and positive urine test dated 5/19/08. Grade C Violation, 7B1.1(a)(3)(b).
2. Ingles used methamphetamine on or about 5/18/08 as evidenced by the admission form
and positive urine test dated 5/23/08. Grade C Violation, 7B1.1(a)(3)(b).
3. Ingles used methamphetamine on or about 6/16/08 as evidenced by the positive urine test
on that date. Grade C Violation, 7B1.1(a)(3)(b).
4. Ingles used methamphetamine on or about 6/26/08 as evidenced bythe positive urine test
on that date. Grade C Violation, 7B1.1(a)(3)(b).
