PETITION FOR ACTION ON CONDITIONS OF PRETRIAL RELEASE
COMES NOW ALMA L. LOPEZ, PRETRIAL SERVICES OFFICER, ofthe Court presenting
an official report upon the conduct and attitude of releasee: Kristina Nicole Ingles, who was
released by the Honorable Lawrence O. Anderson sitting inthe Court at Phoenix, on the @1 day
of May, 2007, who fixed bail at $ O.R and imposed the general terms and conditions of bail
release adopted by the Court and also imposed special conditions as follows:
PLEASE REFER T0 THE ORDER SETTING CONDITIONS OF RELEASE
RE SPECTFULLY PRESENTING PETITION FOR ACTION OF COURT FOR CAUSE AS
FOLLOWS:
ALLEGED VIOLATION:
1. The defendant shall refrain from excessive use of alcohol and not use or possess any narcotic
or other controlled substance defined by 21 USC 802 unless prescribed for defendant by a licensed
medical practitioner in the course of his/her legitimate medical practice. The defendant shall
submit to drug/alcohol testing, including breathalyzer testing as directed by pretrial services.
The defendant failed to submit to urinalysis on May 4, 7, and I0, 2007.
2. The defendant shall immediately advise pretrial services, the court, defense counsel or the US.
attorney in writing ofchartge in address/telephone number.
The defendant failed to advise pretrial services ofchanges in address.
