Modification of Supervised Release Conditions
l recognize that l have a statutory right under Rule 32.1, Federal Rules of Criminal Procedure, to
assistance of counsel (appointed or retained) and a modification hearing in connection with:
I] any unfavorable modification of my conditions of probation. 18 U.S.C. 3553(c).

I hereby voluntarily waive my right to retained and appointed counsel and statutory right to a hearing
as indicated above, and agree to the following modification of my conditions:
7. You shall participate in a Home Confinement Program with electronic monitoring under the
sanction of detention to include alcohol monitoring for a period of up to 180 days. You shall
contribute to the cost of electronic monitoring in an amount to be determined bythe probation
officer.
8. You shall reside and artici ate in a residential re-ent center or 12-ste halfwa house for
