Effective immediately, paragraphs two and three of Local Rule E.D. Ky. LBR, 1007-2 are amended to read as follows: Rule 1007-2. MAILING ­ LIST OR MATRIX The mailing list matrix submitted with a chapter 7, chapter 12 or chapter 13 petition shall include the address of the IRS or the Commonwealth of Kentucky, Revenue Cabinet, only if the debtor owes taxes to those entities. The address to be used for IRS is: Internal Revenue Service P.O. Box 21126 Philadelphia, PA 19114 The mailing list matrix submitted with a chapter 11 case shall contain the address of the Internal Revenue Service as follows: Internal Revenue Service P.O. Box 21126 Philadelphia, PA 19114 whether or not the debtor is indebted to such taxing entity.
