Permit Applications for Recovery Wells
In order to complete the evaluation process for recovery wells, the following items need to be addressed. 1. Please describe briefly each aspect of the remediation plan, i.e., extraction, recovery, reinjection, and address the type of treatment, i.e., air stripper, venting, sparging, etc. You may include a diagram of the process. 2. On the attached form, please provide the proposed average annual amount of ground water to be consumed (i.e., through evaporation) in acre-feet. It is important to know the amount of water diverted, as well as the estimated amount that will be consumed. 3. There is a 600 foot spacing requirement between the recovery well(s) and any other producing well pursuant to C.R.S. 37-90-137(2). Please determine that your well conforms to this requirement and provide a statement affirming this, or obtain a waiver of the 600 foot spacing requirement from any well owners within the 600 foot radius. 4. The application fee for recovery wells is $100 per well. Please submit this fee with the applications. Submitting the above requested information with your recovery well applications is appreciated and will allow your applications to be processed in a timely manner. Thank you for your understanding and cooperation. If you have any questions, please call this office at (303) 8663587, Monday through Friday, between 9:00 a.m. and 4:00 p.m.

ESTIMATE OF GROUND WATER CONSUMPTION BY RECOVERY WELL
A) RATE OF PUMPING B) TIME OF OPERATION C) TOTAL ANNUAL APPROPRIATION D) CONVERSION TO ACRE-FEET E) F) PERCENT OF DEPLETION TOTAL ANNUAL CONSUMPTION
GPM MINUTES/YEAR GALLONS PER YEAR ACRE-FEET

%
ACRE-FEET

1) 2)

Enter the RATE OF PUMPING in gallons per minute, for example 10 gpm. Calculate how many minutes per year the well will be pumped: 24 hours per day x 365 days per year = 525,600 minutes/year. Multiply A) by B): 10 gpm x 525,600 minutes/year = 5,256,000 gallons/year. Divide C) by 325,850 gallons/acre-feet to convert from gallons to acre-feet. In this example, 5,256,000 gallons/year รท 325,850 gallons/acre-feet = 16.13 acre-feet/year. This is the amount of water diverted per year through the remediation system. Determine the PERCENT OF DEPLETION using energy balance methods or empirical methods, i.e., manufacturer's estimates. Say the process utilizes an air stripper and the percent of depletion is determined by manufacturer's estimates to be 2.5%. To calculate the TOTAL ANNUAL CONSUMPTION, multiply D) by E). In our example, 16.13 acre-feet/year x 0.025 = 0.40 acre-feet/year. Preparer should fill out the information requested below. Indicate the Receipt No(s). if known.
