PROBLEM LIST:
1. Stage Il E (L-positive nodular sclerosing Hodgkin's disease).
2. Disorder of iron metabolism.
3. Gout
S: Richard has now had chemotherapy on 3 occasions. Vlhth his last treatment, he was found to be slightly neutropenia and
received a dose of Neulasta from coverage. A few days after that, he had problems with severe bone pain. This has now
resolved. He tells me his gout has improved but he has not yet started back on the allopurinol. He no longer has difficulty with
rash. He had a cough for a few days but this has resolved as well. He continues to work.` His wife is going to be induced
tomorrow.
O: BP 122/70, heart rate of B4, WT 206. Face is symmetric. Sclerae anicteric. Orophaiynx is clear. Neck without adenopathy.
Lungs are clear to auscultation and percussion. Heart is regular, no murmur. Abdomen is soft and nontender, no mass or
organomegaly. Extremities are without edema.
DATA: Labs noted.
A/P: Richard is tolerating treatment very well. He will complete cycle #2 on Friday and I want to restage him with a CT of the
chest a week after that. Unless he has had an amazing response, he will continue with a total of 4 cycles of chemotherapy and
then proceed to radiation. I do not want to give him anymore Neulasta after chemotherapy but he may beneht from intemiittent
Neupogen as necessary.
