(Name of Respondent) It appearing that the court will have to make an Order for Child Support and the Court needing accurate information concerning both the parties gross monthly income, the Petitioner is directed to, along with any other appropriate pleadings, serve upon the respondent, a copy of: 1) This Order Re: Support form 50: 2) Support Form 50-A (Instructions for Completing the Parent's Worksheet for Child Support Amount) and 3) Support Form 50 (Parent's Worksheet for Child Support Amount). It is Ordered that Petitioner shall, within 30 days, complete and file a Support Form 50, Parent's Worksheet for Child Support amount and Respondent shall, within 30 days, complete and file a Support Form 50, Parent's Worksheet for Child Support Amount, or Petitioner and Respondent shall, within 30 days, complete and file a Joint Parent's Worksheet for Child Support Amount with the Clerk of Superior Court, P.O. Box 7000, Kingman, AZ, 86402-7000. Even if Respondent does not wish to contest any item of the Petition for Dissolution, the Respondent must complete and file Support Form 50, Parents Worksheet for Child Support Amount.
