DOCUMENT CHECKLIST
APPLICATION FOR CANADIAN CITIZENSHIP UNDER PARAGRAPH 5(2)(a) MINORS (UNDER 18 YEARS OF AGE) Send the following documents with your application. Check each box once you enclose the item. Failure to provide the necessary documents will result in delays in the processing of your application. Form 1 Application for Canadian Citizenship - Minors (CIT 0003) - completed, signed and dated Photocopies of identity and immigration documents Do not send original documents of the following as they will not be returned promptly. Note: If supporting documents are not in English or French (for example, a birth certificate issued in another country) you must supply a photocopy of each original document, a translation and an affidavit from the person who completed the translation. Translations from family members are not acceptable. 1 Your child's Canadian immigration record/paper (either your child's Record of Landing (IMM 1000) or Confirmation of Permanent Residence (IMM 5292), and both sides of your child's Permanent Resident Card (PRC), if your child has one. Your child's birth certificate (see the guide for instructions on Quebec issued birth certificates) In the case of an adopted child send: Your child's Adoption order In the case of an adopted child living in Quebec send: "Jugement d'adoption"; or "Jugement sur requête en adoption"; or "Reconnaissance de jugement d'adoption"; or "Certificat d'inscription d'adoption"; or "Attestation d'adoption"; or "Lettre d'attestation d'adoption". Two (2) pieces of your child's personal identification (For example, a Canadian health insurance card, a school record, a hospital or immunization record or the page of a passport containing his or her photo and personal information.) If applicable, proof of the child's parent's Canadian citizenship (For example, a certificate of Canadian citizenship or a Canadian birth certificate). If a legal guardian is applying on behalf of the child, provide legal documentation proving guardianship Photos Two (2) citizenship photos (refer to the form Citizenship Photograph Specifications - CIT 0021) If the child is 14 years of age or older, the child must sign his or her name in the white area below each of the photos. Do not print. 1 If the child is under 14 years of age, leave the signature strip blank and print the child's name on the back of the photo. Put both photos of the child in an envelope. Do not staple, glue or otherwise attach the photos directly to the application. Fee 1 Copy 2 of the Receipt form showing the amount paid No other form of payment is acceptable.
